All tax forms will be sent to you directly from the payers (the unions that issue your payments to you throughout the year). They will be mailed to the payment address that is associated with your Tax ID number. If you’ve provided services to an EAP client and have received payments directly from MHC, you will be receiving your tax form from MHC instead of, or in addition to, the forms you receive from each payer. Because MHC and our payers are aware of the importance of being organized and prepared for tax season, rest assured that we are all working hard to guarantee that your tax forms will be mailed to you in a timely manner.
